The notes are unlisted, in registered form, are guaranteed by USI and are repayable on 30 September 2015. FO Advisors LLC is a wholly owned subsidiary of FO-FM LLC of 4571 Stephen Circle, N.W., Suite 200, Canton, OHIO 44718, United States of America ( FOFM ). FOFM is a company in which Todd Lensman owns 65% and Henry S Belden IV owns 35%.
